Best for: Families needing ongoing, high-touch senior family planning and care oversight.  Peace of mind that someone is advocating and looking out for your loved one.  Family circumstances may include busy family life, children working, or out of town.

Includes:

• All services from the Compass Consultation Package 

• Weekly client and/or family check-ins – reports completed by email.  Including weekly care reviews and status reports, 2 hrs including travel time – visit to in city residence

  • Ongoing care plan refinement and implementation

• Attendance at virtual or local medical appointments (maximum allotment applies)

• Coordination with multi disciplinary care providers, legal/financial representatives

• Unlimited Q&A emails

• 6 hours/month of Case Management (calls, documents, research, team meetings)

• Priority scheduling and dedicated access for my services

• Family Communication Tool completed and updated as needed

• Health system navigation and integrative health provider collaboration

Advocate Consultation Package

Add-On Options : 

• Hospital visits – within Calgary city limits, $150/hr, minimum charge 2 hrs.

• Facility Tours such as assisted living residence, long term care, specialized care residence $150/hr including travel.

• Additional Collaborative care in person meetings $150/hr including travel time.

• Assistance executing Roadmap action items & Key Recommendations beyond 6 hours/month, $150/hr.

• Emergency consultation (within 24 hours): $100 surcharge

• Standalone Care Plan Creation: $400 (includes one-90 minute consult and written care plan)

• Standalone Family Communication Tool: $500 (includes one-90 minute consult plus tool).  Ideal for filling out endless intake forms, providing history to medical providers, ensuring that family has a clear understanding of the complete medical picture.
