Crisis Response Consultation Package

• Hospital visits – within Calgary city limits, $150/hr minimum charge 2 hrs.

• Facility Tours such as assisted living residence, long term care, specialized care residence $150/hr including travel.

• Collaborative care in person meetings $150/hr including travel time.

• Assistance executing Roadmap action items & Key Recommendations $150/hr.

• Building complete Family Communication Tool $250

  • Ideal for filling out intake forms, providing history to all care providers, and ensuring that family has a clear understanding of the complete medical picture.

• Customized Care Plan development $150/hr.
